• Form - Borang Permohonan Pengesahan Baki Pelaburan
• IC of Unit holder or Beneficiary – original and photocopy
• Document as proof of the relationship between beneficiary and the deceased. (For deceased unit holder only)
  Eg: Marriage certificate (if husband/wife) or Birth certificate (if children/sibling)
• Death certificate/Burial permit (For deceased unit holder only) – original and photocopy
• Processing Fee (Please refer list of Service Charge)
