- Cheque - Original and photocopy (for outdated cheque/ wrong Infomation on cheque)

ASNB Headquarters Kuala Lumpur or any ASNB Offices.
Note :-
- Only application for replacement of cheque which has not been sent to the Registrar of Unclaimed Moneys (“Registrar”) will be processed by ASNB.
- For application whereby the cheque had already been sent to the Registrar, ASNB will only issue confirmation letter and UMA 7 Form to the applicant.
- The applicant will have to submit the original completed UMA 7 Form, original confirmation letter (from ASNB), photocopy Identity Card/passport and photocopy of a valid passbook (bank account) or bank slip. For beneficiary claims supporting documents i.e. Letter of Administration (probate/Surat Pusaka Kecil/Court Order/ Surat Kuasa from Amanah Raya Trustee Berhad) must be attached.
