REPLACEMENT / REISSUANCE OF CHEQUE

  • Form - Tuntutan Cek Gantian
  • Identity Card – Original and photocopy
  • Cheque - Original and photocopy (for outdated cheque/ wrong Infomation on cheque)
  • Processing Fee (Please refer list of Service Charge)

Where to Apply
ASNB Headquarters Kuala Lumpur or any ASNB Offices.

Note :-

  1. Only application for replacement of cheque which has not been sent to the Registrar of Unclaimed Moneys (“Registrar”) will be processed by ASNB.
  2. For application whereby the cheque had already been sent to the Registrar, ASNB will only issue confirmation letter and UMA 7 Form to the applicant.
  3. The applicant will have to submit the original completed UMA 7 Form, original confirmation letter (from ASNB), photocopy Identity Card/passport and photocopy of a valid passbook (bank account) or bank slip. For beneficiary claims supporting documents i.e. Letter of Administration (probate/Surat Pusaka Kecil/Court Order/ Surat Kuasa from Amanah Raya Trustee Berhad) must be attached.
