BENEFICIARY CLAIMS OF DECEASED UNITHOLDER’S ACCOUNTS – TOTAL INVESTMENT BALANCE OF RM 10,000 AND BELOW

Beneficiary of the deceased unit holder (Child/ Husband/ Wife/ Parents/Siblings/ Grandfather/Grandmother)

Forms and documents required

• T1 Form – Borang Tuntutan Baki Unit Pelaburan
• Death certificate/Burial permit – original and photocopy
• Identity Card of the beneficiary- original and photocopy
• Investment book and/or investment certificate of the deceased (if available)
• Document as proof of the relationship between beneficiary and the deceased - original and photocopy
  Eg:
1. Marriage certificate/Birth Certificate of children (if husband/wife) or
2. Birth certificate (if children/sibling/grandfather/grandmother) or
3. Government Pensioner Card (if husband/wife) or
4. Official document from JPN for substitution of Birth Certificate (muslim and non muslim) and Marriage Certificate (non muslim) or
5. Official document from Pejabat Agama Islam for substitution of Marriage Certificate for Muslim or
6. Official document from Jabatan Hal Ehwal Orang Asli that verifies the relationship between beneficiary and the deceased unitholder or
7. Official document from Mahkamah Anak Negeri Sabah for substitution of Marriage Certificate

If beneficiary and document required is not available, please submit one (1) of the following documents (original and photocopy):-

1. Faraid Distribution Certificate or Letter of Administration from Syariah Court or
2. Letter of Administration from High Court Civil or
3. Perintah Pembahagian Harta Pusaka Kecil Pejabat Tanah & Daerah or
4. Surat Akuan dan Arahan Amanah Raya Berhad; or
5. Letter of Administration from Mahkamah Anak Negeri Sabah or
6. Letter of Administration from Mahkamah Anak Negeri Sarawak
