Forms and documents required
- T1 Form – Borang Tuntutan Baki Unit Pelaburan
- Death certificate/Burial permit – original and photocopy
- Identity Card of the beneficiary (s)/administrator- original and photocopy
- Investment book and/or investment certificate of the deceased (if available)
- Original and photocopy of Letter of Administration from:-
- High Court Civil/High Court Sabah/Mahkamah Anak Negeri Sabah/Syariah Court Sabah/High Court Sarawak/Mahkamah Anak Negeri Sarawak
- Surat Akuan dan Arahan Amanah Raya Berhad; or
- Perintah Pembahagian Harta Pusaka Kecil Pejabat Tanah & Daerah
