A. Document Needed
Copy of applicant Identification Card and Birth Certificate/ Form W/ Certificate of Citizenship OR MyKad/MyKid Print-Out that stated the applicant race.
B. Additional Document Needed
1. Mix Native of Sabah/Labuan/Sarawak Only
Confirmation letter from Pejabat Hal Ehwal Anak Negeri Sabah (PHEANS)/ Mahkamah Anak Negeri Labuan / Sijil Anak Negeri from Mahkamah Bumiputera Residen Sarawak need to be obtained.
