i. ACCUMULATED BALANCE OF INVESTMENT OF RM10,000 AND BELOW

Forms and Documents Required

• T1 Form - Borang Tuntutan Baki Unit Pelaburan - with RM 10.00 stamp duty duly nullified at the Stamp Office or at Session Court/ Magistrate/ Commissioner of Oath
• BS 10 Form - Borang Penukaran Penjaga Berdaftar
• Death certificate/Burial permit – original and photocopy
• IC of the new Registered Guardian - original and photocopy
• IC and /or Birth Certificate of the Minor - original and photocopy
• Investment book of Akaun Remaja/Bijak (if available)
• Document as proof of the relationship between new Registered Guardian and Minor – original and photocopy
  Eg: Birth certificate

ii. ACCUMULATED BALANCE OF INVESTMENT ABOVE RM10,000

Beneficiary of the deceased unitholder or new Registered Guardian or administrator appointed with Letter of Administration

Forms and documents required

• BS10 Form – Borang Penukaran Penjaga Berdaftar
• TI Form - Borang Tuntutan Baki Unit Pelaburan
• One (1) of the following documents :-
  1. Letter of Administration from Mahkamah Tinggi Malaya/ Mahkamah Tinggi Sabah/Mahkamah Anak Negeri Sabah/Mahkamah Rendah Syariah Sabah/Mahkamah Tinggi Sarawak/Mahkamah Anak Negeri Sarawak; or
  2. Surat Akuan dan Arahan Amanah Raya Berhad; or
  3. Perintah Pembahagian Harta Pusaka Kecil Pejabat Tanah & Daerah – original and photocopy
• Death certificate/Burial permit – original and photocopy
• IC of the new Registered Guardian - original and photocopy
• IC and/or Birth Certificate of Minor - original and photocopy
• Investment book of Akaun Remaja /Bijak (if available)
