• BS 10 Form - Permohonan Penukaran Penjaga Berdaftar
• IC and/or Birth Certificate of Minor – original and photocopy
• IC and Birth Certificate of the former Registered Guardian and the new Registered Guardian – original and photocopy
• IC of Mother/Father of the new Registered Guardian – original and photocopy
• Investment book of Akaun Remaja/Bijak (if available)
