Both existing Registered Guardian and new Registered Guardian

Forms and Documents Required

• BS 10 Form - Borang Penukaran Penjaga Berdaftar
• IC and/or Birth Certificate of Minor – original and photocopy
• IC of existing Registered Guardian and new Registered Guardian – original and photocopy
• Investment book of Akaun Remaja/Bijak (if available)
• Document as proof of the relationship between new Registered Guardian and Minor.
  Eg: Birth certificate

Where to Apply
ASNB Headquarters Kuala Lumpur or any ASNB Offices.

Note: - Both existing and new Registered Guardian must be present during application.
