EPF MEMBER'S INVESTMENT SCHEME

Applicant
Individual registered unitholder aged above 18 and below 55 years old and meets all the conditions and requirements imposed by EPF on the EPF investment withdrawal scheme.

 Forms and Documents Required

• ASNB 21 Form - Pelaburan Tambahan (ASNB 21)
• KWSP 9N (AHL) Form - Permohonan Pengeluaran Pelaburan Ahli
• The latest EPF statement
• Identity Card – Original and photocopy (both front and back of the card on the same page of A4 paper, with proof of right and left thumb print below the copy of the IC)

ASNB Headquarters Kuala Lumpur or any ASNB Offices.

 Note: - Application is subject to :-

(i) Investment in ASNB Funds with EPF features only
(ii) The applicant must be a registered unitholder
(iii) Available units for subscription in each fund
(iv) Terms and conditions imposed by EPF

EPF Smart Kiosk is now available at UG Balai PNB, Jalan Tun Razak to ease EPF member's to print statement immediately.
