- BS 2 Form - Penukaran Bentuk Buku ke Sijil
- Investment book
- Identity Card of Unit holder/Registered Guardian - original
- Processing fee (Please refer list of Service Charge)

ASNB Headquarters Kuala Lumpur , any ASNB offices or any ASNB Agents.
Note:-
- For application made at ASNB Headquarters Kuala Lumpur or ASNB offices , investment certificate will be issued immediately.
- For application made at ASNB Agents, investment certificate will be posted to the address as per ASNB's record.
