A. Correction of unitholder’s Identity Card number/name

• BS 4 Form – Penukaran Butir-butir Peribadi
• Identity Card – original & photocopy
• Birth Certificate – original & photocopy (if applicable)

B. Correction of Registered Guardian’s Identity Card number/name

• BS 4 Form – Penukaran Butir-butir Peribadi
• Identity Card – original & photocopy

C. Change of unitholder’s/Registered Guardian’s Identity Card number

• BS 4 Form – Penukaran Butir-butir Peribadi
• Identity Card – original & photocopy
• Birth Certificate – original & photocopy (for change of Birth Certificate number to Identity Card number)
• Confirmation letter from employer on the change of IC from civilian to armed forces/police – photocopy (if applicable)
• Confirmation letter from employer on the old IC no. for armed forces/police and new IC no. for armed forces/police – photocopy (if applicable)
• Confirmation letter from Jabatan Pendaftaran Negara – photocopy (if applicable)

D. Change of unitholder’s/Registered Guardian’s name

• BS 4 Form – Penukaran Butir-butir Peribadi
• Identity Card – original & photocopy
• Birth Certificate – original & photocopy (if applicable)
• Confirmation letter from Jabatan Pendaftaran Negara (if applicable)
• Confirmation letter from Jabatan Agama Islam – photocopy (if applicable)

E. Change of unitholder’s address

• BS 4 Form – Penukaran Butir-butir Peribadi

Where to Apply

1. Application for change of address and IC of unit holder/Registered Guardian can be made at ASNB Headquarters, Kuala Lumpur , ASNB offices and ASNB Agents.
2. Application for correction of name and/or IC no of unit holder/Registered Guardian will be processed at ASNB Headquarters, Kuala Lumpur only.
3. Application for change of Unitholder/ Registered Guardian name will be processed at ASNB Headquarters, Kuala Lumpur only.
