ASNB Applications & Forms
Registration
Used for opening a unitholder account. The form must be read together with the relevant Prospectus or Product Highlights Sheet (PHS). Forms are available at ASNB branches and appointed agents, with electronic copies and guidelines also provided on the official ASNB website.
Salary Deduction Scheme (SPG) Application
Invest automatically through regular salary deductions arranged by your employer by completing the Salary Deduction Authorisation Form (SPG) or the Salary Deduction Facility Form (PPS-SPG), which includes variations applicable for employers and family members.
Estate Claim Applications are required for claims on behalf of deceased unit holders
This involves the Estate Claim Form (T1), a completion guide, and additional information forms for administrators, heirs, or beneficiaries where applicable. Claims related to Trust Gift (Hibah Amanah) or Trust Declaration should refer to the respective dedicated sections.
Trust Grant Application
Form: PH1A and supporting documents listed in the checklist; a filling guide is also provided.
Application of Detailed Statement
Request a detailed statement of transactions and/or holdings for a selected period.
Form: Detailed Statement Application Form and, if submitted by a representative, a Power of Attorney as specified.
Channel: ASNB branches or appointed agents; details on requirements and locations are available on the services page.
Fatca & CRS Declaration
Complete the FATCA & CRS Declaration Form to comply with cross-border tax reporting requirements. This form is used during account opening or information updates at the counter. Please refer to the FATCA & CRS compliance guidelines for details.
Trust Gift (Hibah Amanah)
Applications require Form PH1A together with the supporting documents listed in the checklist, which includes a guide to assist applicants.
Others
Depending on the type of transaction, the following supporting forms may be required:
Personal Information Update – Updates to address, email, phone number, or bank account details.
Fund documents and application forms accompanying the Prospectus/PHS – Printed copies are available at ASNB branches or appointed agents.
Forms / notices related to myASNB digital services (e.g. activation or verification).
